Alumni must fill out the attached form and either fax, mail, or send it to Bishop Ward to request a copy of a transcript. Transcript requests should be sent to Kay Byard (kbyard@wardhigh.org or 913.371.1201). There is a fee of $2 for each transcript. Click the link above to open a PDF of the form.

Phyiscal Form for Student Athletes
The Kansas State High School Association (KSHAA) must have physicals on file before any student athlete can participate in any sport. They cannot try out without a doctor’s approved physical form. School Transportation Release form
