Physical Forms for Student Athletes
The Kansas State High School Association (KSHAA) must have physicals on file before any student athlete can participate in any sport. They cannot try out without a doctor-approved physical form. Click here to download and print a copy of the state required physical form. The form is in PDF file format.
Athletic physicals for the 2013-14 school year must be dated after May 1, 2013.

The OrthoKC doctors and SAFE-K organization are hosting $15 athletic participation physicals for next year. If you are a parent of an athlete, this is a great opportunity to complete your pre-participation sports physicals for the 2013-14 school year.

Cost: $15. Cash, money order, debit and credit cards are accepted.
Appointments are not required. OrthoKC is located in the Providence Medical Building at 8919 Parallel Parkway, Suite 270. You can print a physical form here, pick one up from the school office, or physical forms will also be available at the OrthoKC office. If you are able to attend, please identify yourself as a Bishop Ward student and a portion of the cost of the physical will be donated back to our school. Parent attendance is RECOMMENDED.
