Transcript Request Form


All transcript requests must be made in writing and require the signature of the transcript owner to legally release the record. Transcripts are not released through phone calls, voicemails, or emails. The transcript request form must be completed entirely and include the signature of the transcript owner to release the record.

Please see the link for the PDF file that you must download, print, complete and return to the Registrar's Office. Once the form is completed you may scan it or take a clear picture of it and email it to the Registrar. If you have the capability to fill out the PDF electronically and provide an electronic signature, that form can be accepted as a legal request. Transcript request forms are also available in the Main Office at school, or a request form can be mailed or faxed to you.

In following the Federal Right to Privacy Act (FERPA), transcripts are not released to anyone except the transcript owner. This restriction includes, but is not limited to: parents/legal guardians, spouses/partners, family members, post-secondary institutions, NCAA/NAIA, military, employers, unions, verification companies, or for court/legal purposes. If the transcript owner wants to allow his/her transcript to be released to anyone other than himself/herself, then the transcript owner must provide written, authorized consent, with a signature and date, to the Registrar to legally release the record.

There is a $7.00 per transcript fee. Accepted payment methods are: cash, money order, debit or credit card. Payment can be accepted in person at Bishop Ward with the Main Office or with the Registrar or over the phone using a debit or credit card with the Registrar. Payment will not be accepted until the transcript request form is submitted. You may not pre-pay over the phone. Transcripts will be sent for free for one year (up to five (5) transcripts) for members of the most recent graduating class, the Class of 2024. While the fee is waived for the Class of 2024, a transcript request form must still be completed and submitted to the Registrar's Office to release the record.

Please note: our school seal does not appear on an electronic transcript. Some employers, colleges and universities, and the military will not accept a transcript as an "official" transcript without the seal on it. However, most organizations will accept an electronic transcript as "official" if it is sent directly from the Registrar. Additionally, a hard copy of the transcript can also be mailed at no additional cost to the requestor, if needed. You will need to specify whatever method you choose on your request form. If you want to pick up an "official" transcript, it will be provided in a sealed envelope. As long as you keep the envelope sealed, it will be considered official. Once it is opened, it is no longer considered official and may not be accepted.

Transcripts are processed daily as they are received when school is in session. Any requests received after 2:00p.m. will be processed on the next business day. If a request is submitted and school is not in session or closed for a break, it will be processed as soon as possible when the Registrar is back in the office.

If you have any obligations to Bishop Ward High School, your transcript will not be released until the obligation is completely fulfilled.

If you are a current Bishop Ward student or the parent/legal guardian of a current Bishop Ward student, you need to contact your Guidance Counselor for any transcript requests.
